In this case, an elderly patient in hospital with a known bowel obstruction collapsed.

The hospital failed to resuscitate her as an emergency or to proceed with a laparotomy and when they did proceed with surgery, performed the wrong operation.

Tragically, she died a short time later. With proper treatment, it is expected that she would have survived.
